I am paneled with some insurance carriers in the State of Hawai'i. If I am paneled with your insurance, you would only be responsible for the cost of the copay at the time of the appointment and your insurance will be bill accordingly.

If I do not take your insurance, I am happy to provide you with a Superbill to submit to your insurance carrier. Please check with your insurance carrier for reimbursement rates. If you have a Flex Spending Account for medical services, I can also provide you with a Statement for submission for reimburse. However, in both of these instances, you would be responsible for full payment at the time of service.
